Prior to your first visit, please complete your registration forms through our Patient Portal or by downloading the forms.

To access the Patient Portal:

  • Click the Patient Portal button below.

  • Log in using the username and password from office staff / email.

  • Within the portal, click the “My Health” icon at the top of the screen and fill in the information.

If you prefer to download the forms, click the button below to print forms and fax them to 212.924.7557, email them to info@drlancebrown.com, or bring them with you to your first office visit.
